Job Description

Position Summary

Clinical pharmacist provide pharmaceutical care to patients by monitoring drug therapies, reviewing medication orders, providing drug information; In addition, clinical pharmacists provide clinical guidance to pharmacy and health care staff.

Summary of Main

Duties and Responsibilities.

 

  • Consistently complies with UHS policies, procedures and practices. 
  • Completes and maintains all regulatory requirements including: licensure and certification and other mandatory training within established time frames. 
  • Able to evaluate, identify, prevent and manage drug-related problems.
  • Assume responsibility for patient-specific pharmacotherapy plan to ensure optimal outcomes.
  • Assists medical staff with the selection of  evidence- based drug therapy and patient monitoring
  • Reviews medications orders and patient medication profiles for appropriateness of therapy.
  • Monitors for drug interactions, duplication of therapy, allergies, adverse reactions, appropriate formulary alternatives and identifies medications.
  • Recommends changes in medication therapy based on appropriateness and medication management initiatives (example: policies)
  • Demonstrates sound judgments, good decision making and conclusions
  • Calculates and confirms pharmaceutical dosage calculations                  (pediatric doses, IV admixtures, chemotherapy, etc.).
  • Identifies and capable of using resources to ensure safe medication practices.
  • Report and investigate adverse drug reaction and medication errors to ensure safe medication use system.
  • Maintains professional knowledge by attending lectures, seminars or on-line education units.
  • Participates in departmental competency examinations.
  • Shares drug information accurately with physicians and other health care workers.
  • Responds to physician, health care workers and patient drug related questions appropriately.
  • Researches drug information using appropriate resources.
  • Provides in- service education to nurses, pharmacy staff and other health care worker as needed.
  • Represents pharmacy in appropriate committees.
  • Maintain proper documentation on patient profiles or prescriptions.
  • Maintain progress notes and care plans documentation.
  • Responsible for documenting all intervention in appropriate format.
  • Actively participates in patient care rounds with other members of the healthcare team
  • Counsels patients/nurses with clear and accurate instructions.
  • Accepts and adjusts to changes in departmental procedures.
  • Effectively communicate with physicians and nurses by delivering timely information pertaining to pharmaceutical care.
  • Other related duties as assigned. 

Position

Requirements/Qualifications

Education: BSc or Doctor of Pharmacy (Phar.D.) degree from an

                   accredited college of pharmacy.

Experience: BSc. Degree at least 3 years of experience as a clinical pharmacist.  Doctor of Pharmacy degree (Pharma D) at least 1 year experience as a clinical pharmacist.  Post baccalaureate Pharm D at least 1 year experience as a clinical pharmacist.  Post baccalaureate M.S. at least 1 year experience as a clinical pharmacist.

Desired: a post graduate qualification in clinical pharmacy

               (Preferably Master Degree)

 

Licenses/Registration

  • Must have a valid UAE MOH Evaluation.

 

Position criteria

  • Dispensary experience working in a complex healthcare environment such as hospital or large clinical setting.
  • Experience in meeting KPI’s
